Care Coordinaton

Providing efficient and effective client care is paramount to our team. Care coordination takes into account individual client needs and creates actionable goals to address them. This includes internal referrals to our many services and professionals or external referrals to our community partners.

Internal services include:

Addictions counselling
Case management
Psychology consultation referrals to our internal psychologist
Patient navigation for Hepatitis C treatment and testing, HIV testing, as well as PEP and PrEP
ODSP clinic in partnership with Belong Ottawa
Tax form clinic in partnership with Causeway

External services include:

Specialist referrals by request
Liaison support services for local resources such as shelter and food supports
Bridging care to Withdrawal Management Services or in-patient facilities
