
Case Management
Our Case Management services are available at our ByWard, Vanier and Cornwall locations to support individuals receiving care at Recovery Care who may need assistance navigating social, health, and community-based systems. Providing efficient and effective client care is paramount to our team. Case Management takes into account individual client needs and creates actionable goals to address them.
Clients may receive Case Management support if they are seeking assistance with housing, accessing community resources, or addressing other social needs.
Please note: Case Management is an internal service available only to existing Recovery Care patients and is not a standalone service. We do not accept external referrals specifically for Case Management support.
What We Offer
Our team provides practical, client-centered support, including:
- Assistance with identifying needs and setting goals
- Case management and patient navigation support
- Connecting clients with community partners to help bridge gaps in care
- Support with accessing programs and services such as Canadian Mental Health Association (CMHA), The Salvation Army, RAAM clinics, detox services, and treatment centres
- Informal counselling and de-escalation support, with referrals to formal counselling services as needed
- Support accessing housing resources, community supports, and other social services
What Is Not Included
To ensure clarity, Case Management services do not include:
- Assistance with lease signing or legal agreements
- Searching for private-market rental housing on behalf of clients
- Intensive case management or therapeutic counselling services
- Ongoing support for individuals who already have an established community support worker
